For graduating from high school or are college undergraduate students who:
  • Face the daily challenges of living with a chronic disease or condition.
  • Have an ongoing medical condition or illness.
  • Have struggled to deal with the impact of the disease, and are committed to pursuing higher education.
  • Are from the Western New York region attending college anywhere in the country.
  • Demonstrate financial need
  • Minimum 2.5 GPA
